Google publishes MedGemma medical VLM family in Nature Medicine

TL;DR

  • MedGemma is a collection of medical vision-language foundation models based on Gemma 3, with 4B and 27B multimodal variants plus a 27B text-only model.
  • The paper reports gains over base Gemma 3 of 2.6–10% on medical image QA, 15.5–18.1% on chest X-ray finding classification and 10.8% on agentic evaluations.
  • The release ships with MedSigLIP, a medically tuned vision encoder derived from SigLIP that the authors say matches or beats many specialized medical image encoders.

Google has published MedGemma, a family of open medical vision-language models built on Gemma 3, in Nature Medicine. The paper reports improvements over the base Gemma 3 models of 2.6–10% in medical image question answering, 15.5–18.1% in chest X-ray finding classification and 10.8% in agentic evaluations.

The release spans four artefacts: multimodal MedGemma 4B and 27B, a 27B text-only variant, and MedSigLIP, which the abstract describes as "a medically tuned vision encoder derived from SigLIP" that "achieves performance comparable to or better than that of many specialized medical image encoders."

The authors frame the family as a foundation for downstream development rather than a finished clinical system. They write that "fine-tuning MedGemma can be more effective than fine-tuning the base Gemma 3 model for medical tasks, particularly in the setting of limited training data," the pitch aimed squarely at hospital groups and startups without FDA-trial-scale labelled corpora.

Two of the researchers in our Who's Who were already circulating the paper link this week.
